To begin services with CIL, you need to complete our consumer intake form.

Step One: Complete Consumer Intake Form

Complete the online intake form. Or download, print, and return the intake form (PDF), also as Word or large print (Word). Or complete it in person, using the large print intake form.

Step Two: Meet with Our Team

After your consumer intake form is complete, our team will meet with you to discuss your goal.

Step Three: Begin Services

Our team will work with you as you work towards achieving your goal for independence.
